4-H Name & Emblem Guidelines

The 4-H Name and Emblem is a federal mark with protections and standards that exceed that of trademarks and copyrights. Using the 4-H Name and Emblem requires proper authorization. Anyone engaging in unauthorized use or misuse of the 4-H Name and Emblem is subject to federal prosecution.

The U.S. Congress has legislated responsibility for proper management of the 4-H Name and Emblem to the Secretary of Agriculture, who has delegated that authority to the Division of Youth and 4-H housed at USDA, NIFA. The use of the 4-H Name and Emblem is defined through Section 7 in the Code of federal Regulations, Part 8.

About these forms

The forms below are required from clubs per the Wapello County Agricultural Extension fiscal policy. Forms can be filled in digitally and emailed to the office or printed at home. If you would like a hard copy of these forms, please reach out to staff or stop at the office.

Club Voucher Request

This form is required to be refunded for purchasing supplies for the club or allocating club funds. 

In order for your voucher request to be processed , you must complete option A or Option B and include the following:

Option A

1. A completed Voucher Request Form  

2. All detailed receipts and detailed invoices

3. Club minutes approving the exact amount to spend or state not to exceed X dollar amount in minutes

Option B

1. Online Voucher Request-Coming Soon

2. All detailed receipts and detailed invoices (there will be a spot to upload)

3. Club minutes approving the exact amount to spend or state not to exceed X dollar amount in minutes. (there will be a spot to upload)

Club Account Deposit

This form is required to deposit funds into your club account. You will likely use this form in conjunction with a fundraiser.

In order for a deposit to be made, you must include the following:

1. Completed and signed deposit form

Fundraising Request

This form is required if your club wants to hold a fundraiser.

In order for a fundraiser request to be approved, you must include the following:

1. Fundraising Request Form

2. Post-event report (bottom part of the above form)-to be completed within 10 days of the fundraiser. 

3. Fundraiser budget worksheet (reverse side of the above form)

4. Submit this form at least 1 month prior to your fundraiser.

Gift Card Reconciliation

This form is to be filled out by the recipient of the gift card and returned to the Wapello County Extension Office in order for the gift card to be given to the recipient. Gift cards cannot be more than $100 each. 

In order for a gift card to be distributed you must include the following:

1. Completed gift card reconciliation form

2. Club minutes approving the exact gift card amount

Club Budget Worksheet

This form can be completed one time per year. It must be approved at a club meeting and stated in the minutes that it was approved. Retain a copy of this form with your club treasurers notes and send a copy to the Wapello County Extension Office. 

This form can be used in place of club minutes when submitting voucher requests as long as the amount is listed on the budget worksheet. If the amount spent is over the budgeted amount, the club will have to submit minutes with that voucher request stating that additional funds (state amount) are approved to be used.
