Thank you for your interest in SWITCH. SWITCH takes a new approach to school wellness programming by building the capacity in schools to effectively implement an evidence-based, obesity prevention program. SWITCH promotes healthy environments (both in school and at home) but the goal is to help students to ‘switch what they Do, View, and Chew'. The SWITCH program provides a systematic method to help schools develop strategies to plan, implement and operate school wellness programs on their own – with additional support provided through partnership with ISU Extension and Outreach, Iowa 4-H.
Registration is open for the 2018-2019 school year. SWITCH is in the final year of the pilot phase for the SWITCH Elementary program for 4th-5th grades. The funding for this pilot will cover the first 40 schools to enroll. The program is looking for 20-25 returning schools and 15-20 new schools.The SWITCH Middle School program for 6th-8th grades is in its first year of the pilot and will be accepting 12 schools.
Registration for SWITCH is a two-step process. The first is through our online form and the second, is through email and is a document requiring signatures.

Following submission of the online registration form, you will receive a confirmation.
If funded openings are available, the Administrator and Core Team Leader will receive a separate email asking them to sign and return their completed IRB form which will also include tax id number request. You will also be notified if openings are not available.
If a school previously participated, they will not need to complete a W-9. If the school is new to the SWITCH Program, a W-9 will be sent to both Administrator and Core Team Leader for completion. Usually a school business office is equipped to handle this. Please know that a payment cannot be filed until the school has completed and sent in the form
Attendance at the Conference is required for all funded schools to receive stipends. Conference dates are November 8, 2018 for Elementary Schools and November 9, 2018 for participating Middle Schools. Both Conferences are held in Ames.
Finally, do know if all of the paid openings for elementary and middle school have been filled, there is an option to enroll in the “open option”. The open option offers schools to take part in SWITCH program, but the program costs are not waived.
