Click here to find 4-H record keeping forms and help sheets, as well as award applications! In order to be considered for any county awards, all record keeping and award applications must be turned in to the extension office by September 15th!

There are several ways to keep track of your records in 4-H!  Record keeping helps show what you've accomplished throughout your year and provides a year-by-year look at how you're growing through your involvement in the 4-H program.

These are the things that are required in record keeping process:

1. Title Page

2. Yearly Participation Summary

3. Static Goals (if any projects)

4. Livestock Records for each species (if any projects)

5. Applications for awards

These are the things that are optional in the record-keeping process:

1. Photos

2. News Clippings

3. Reflection Sheet AND Static Goal Cards, You may choose one or the other.

4. Additional Records

You can choose to include all of your static goal sheets for each project area OR fill out a record reflection sheet for each project area (ex. if you took 6 photos you only need to fill out one sheet.)

Livestock Record Keeping

Many animals already filled out this record-keeping sheet to exhibit animals at the fair. Please be sure to go through and check you do not already have this filled out!
