This is a suggested order of how your record book should look and pages you should have included. This makes a good check off list.


Junior Awards:

- Leader Statement (on back of County 4-H CHECK Form above)
This is a written statement from your leader telling us about your 4-H club work. This sheet will be removed from your record book after it has been evaluated.

 (Include only if applying for an award.) 
ONE FORM is required for each PROJECT AREA you are applying for and also
ONE FORM for RECORD KEEPING AWARD.
PLUS.... for each project area; complete one of the forms listed below that fits your grade level. (Grade as of May 2022).

Advanced Project Record

(Must be completed for Completion Award) 
This form serves as a summary of your year in 4-H. There is a place to write what activities and events you have done in your club, recognition received, activities you have done at home, in school, and in your community on regular basis. There is also a place to report on projects that you do not complete a Project Record form.

Club Awards - Leaders 

To encourage clubs to achieve excellence in club activities to become involved in county events. 

 

 

 

 

Outstanding Leader & Volunteer Forms 

To recognize a 4-H club leader or volunteer for outstanding service to the 4-H club program. 

To recognize a volunteer for outstanding service to the local 4-H club program. This may be a parent or other community member who has helped the club by driving to events, helping with special projects, sharing talents at workshops, etc. 

 

 

More about Earning an Award

The purpose of awards is to recognize 4-H members in a particular project area who feel they have done a great job that year. This is a self-nominaiton award - YOU DECIDE and YOU do the record book. You don't have to have the champion beef or a project considered for the State Fair, you just need to have set some goals (over the past 2 years that show growth) and met them. 


Members must meet these requirements:
1. Be enrolled in the project area for at least 2 years.
2. Complete project record sheets for the current year and include record sheets from previous years (see below - basic, experienced, or advanced).
3. Complete the 4-H Yearly Summary (see below).
4. Show growth in the project area through setting progressively more challenging goals and meeting them.
You may add as much supplemental information as you want to your project records including picturs, clippings, etc.
Your book should be done neatly and it is best practice to answer the above questions on a computer and print it out.
