SHIPPING INSTRUCTIONS 

1. Place clearly marked sample container in a plastic Ziploc bag (1 sample per bag). 

2. Complete separate order form for each sample and place in a Ziploc bag. 

3. Double check if code on sample and order form matches. 

4. Package well with Styrofoam, packing peanuts, double boxes, crumbled paper, or similar. Insulated shipping containers can be purchased at stores like Staples and Office Depot. Shipping and sampling containers can be requested from the Midwest Grape & Wine Industry Institute. 

5. Include ice packs(double bagged) if your sample requires. 

6. For the shipping declaration, use the phrase: “Research sample containing grape juice and ethanol” for wine samples, or “Research sample containing grape juice” for non-fermented juice samples. 

7. Ship overnight via Fed Ex or UPS to:
