To register, complete the on-line form below. Registration is required to attend this training and is due by Wednesday, October 14, 2009. Registration is limited to the first 25 people.

Training cost is $350 per person. Full-time ISU Extension employees receive a 50% discount ($175). Costs include: handouts, snacks, and lunches.

Payment is due by Oct 21, 2009. Make checks payable to Iowa State University Extension.

Required Manual
Participants are required to bring a SFP 10-14 manual dated April 2007 or newer. Manuals are required and may be ordered for $175 at http://www.extension.iastate.edu/store (SF 0002). If you wish ISU Extension to order a manual for you, $180 will be added to your registration.

Hotel
A block of rooms have been reserved at the Best Western University Park Inn and Suites. The rate is $75.96 for Oct 27-28 and $79.96 for Oct 29. The block of rooms will be released on September 27th so book your room now. When making reservations, please mention the block of rooms is under "Strengthening Families Training”. Note: you must call the hotel directly to reserve a room under the block...the price will not be honored if reservations are booked online.
