Enrollment Forms
For 2008-2009 4-H Year
In order to complete your enrollment process the following three forms along with the $35 Development Fee are due at the Extension Office. All re-enrollments are due on or before December 1st by 4:30 p.m. New enrollments who wish to exhibit at fair are due on or before December 1st by 4:30 p.m. New enrollments NOT wishing to exhibit at fair are due on or before May 15th by 4:30 p.m. Late forms will not be accepted. Postmarks wil not be accepted.

record book to your 4-H leader.
Step 4. Print and complete merit and excellence award
application according to your level.
Step 5. Give application to your leader.
Step 6. If applying for county awards, complete the
appropriate county awards form and follow
the instructions listed.
