Evaluation Instruments 

Powerful Tools for Caregivers class leaders should use the post program Class Evaluation Form included in the Powerful Tools for Caregivers curriculum guide, Appendix 7, pages 9-10. Class leaders should ask participants to complete the evaluation following the last class of the six week series. Class leaders should compile responses from each class series for selected questions requested on the Powerful Tools for Caregivers Resources for Class Leaders: On-line reporting form.
