Evaluation Instruments 

Two evaluation tools have been developed for this program. Impact Evaluation (item 2) should be conducted for significant Retirement Planning programs.

1. Single Session Evaluation (optional)  Retirement Planning Workshop Evaluation (MS Word) may be used for this purpose. Keep this information for your own use.
2. Impact Evaluation – to be conducted for in-depth/sequenced (2+ sessions) programming, using the Retirement Planning Follow-up Evaluation (MS Word) form.

Summarizing and Reporting Impact Data
If any of the statements on the follow-up evaluation are checked, include this participant in 7a) # of individuals who have planned for their retirement on the Families Activity Report and Evaluation Summary.
