Evaluation Instruments 

A Protecting Your Privacy Evaluation Form (MS Word) has been developed for workshops on consumer privacy and identity theft. The evaluation should be given to participants of in-depth/sequenced (2+ sessions) programming.

Summarizing and reporting data
The evaluation asks participants to indicate behaviors before and after attending the workshop. If one or more items are reported as 'no' before attending and 'yes' after attending, the participant would be included in the count for indicator 8a) strengthening consumer decision-making skills on the Families Activity Report and Evaluation Summary.
