Evaluation Instruments 

The U.S. Trustee Office requires that participants complete an end-of-class evaluation form (MS Word). Keep the completed evaluations in your files for possible audit by the U.S. Trustees and to report on ISUE's renewal application as an approved provider.

If this evaluation is used for in-depth/sequenced (2+ sessions) programming, report responses indicated under "Course Results". If "yes" is circled for either of these two statements, include participant in 5a) # of individuals who have improved personal and family management skills on the Families Activity Report and Evaluation Summary.
