A Debt Reduction Follow-up Evaluation (MS Word) has been developed for workshops on credit and debt reduction.  The evaluation should be used at the conclusion of in-depth/sequenced programs (2+ sessions). The follow-up evaluation may be adapted to include additional items that better reflect the content of your workshop. 

Summarizing and reporting data. On the evaluation form, participants are asked whether they have completed any of several activities related to debt reduction.  If one or more of the activities are checked by a participant, the participant would be counted as “yes” and included in the count for item 6a) of the Families Activity Report and Evaluation Summary. Record number of participants who were surveyed and number responding in items 6b) and 6c).
