Listed on the 330 Evaluation Form are indicators that can be used to evaluate behavior changes in clients who participated in programs in nutrition and health. Please select statements that are appropriate for your program efforts and use them in the evaluation form for participants. Evaluate participants at three months post program. When sending the evaluation forms to participants, please work with your office assistant to make sure the mailing follows appropriate postal guidelines.

Expanded Food and Nutrition Education Program (EFNEP) and Family Nutrition Program (FNP) Evaluation
EFNEP and FNP participant data is collected through the EFNEP and FNP reporting system. Only narrative information will be required in EFNEP and FNP for planning and reporting evaluation of the program.

Documenting group work with families not enrolled in FNP

CEEDs, Field Specialists and Program Assistants should document group work done with Food Stamp participants, applicants, and those who are eligible for Food Stamps. Complete the FNP Group Reporting Form for each group that you teach at least one lesson to, but do not officially enroll in the Family Nutrition Program.
