Using a County Code is another way of tracking a county's identification along with the county name. County Codes are used on annual reports, other statistical reporting, and 4-H Blue Ribbon and Fair Management by Martech computer programs.

These forms are provided by the various government agencies and are mailed to you quarterly or annually.
All 4-H forms can be found in the 4-H Forms Notebook, State Fair Handbook, Youth and 4-H Conference Handbook, and the 4-H Recognition Handbook. The forms are replenished by the state Youth and 4-H Office each year.
These forms are provided by your local post office and your area extension education director. Consult both for the appropriate forms. For more information about postal regulations, see Section 4.B.
(Use the form that was revised October 1996, which has a place in the upper left corner for the Federal Agency Cost Code. See Section 4.B for your county's Federal Agency Cost Code.)
If a new county employee is hired, the following forms must be completed:
Reference the "Volunteer Leadership ISOTURE" manual for forms and instructions on how to complete the screening process.
The "Request for Registry Information" form DCI 150 can be obtained at your local Sheriff's office and the "check" is done there.
