Livestock Identification Forms
Due May 15th, 2008!

All livestock ID sheets are due to the Extension Office no later than 4:30 p.m. on Thursday, May 15th!  All 4-H’ers are responsible for turning them in for breeding beef, horses, dogs, dairy goats, dairy cattle, bottle lambs, B/B calves, breeding meat goats, rabbits & poultry. 
4-H’ers who are leasing a horse need to have a Horse Lease Agreement signed and attached to the ID form.  The lease agreement form and other Identification forms are available online at
/mahaska/info/4h/livestock.htm.


Livestock Affidavits Due!
The Livestock Affidavits are Iowa 4-H Animal Care and Management Disclosure Statements, which means they are records of your animal’s medication. 
Affidavits are required for
Horse, Market Lamb, Market Swine, Market Beef & Meat Goats.  These forms are due to the Extension Office by July 14 and for Horses will be due at the Vet Check prior to Fair.


Animal Location Disclosures are due on May 15 for livestock being exhibited at the county fair.
