Power Pay is a program through Iowa State University designed to help you plan a way to pay off balances to credit cards.
Access the necessary forms at:

When you select Power Pay you will be asked to complete a print copy of a data sheet to send to the Iowa State University Extension Financial Planning Clinic, or to complete the form on line. The information you provide will be used to develop a plan to pay off high
rate of interest credit and increase payments to reduce your outstanding balances.
Power Pay forms are also available at your local Extension Office.
