All 4-H'ers will need to complete an Advanced Project Record or an Experienced Project Record (both forms contain the same information, but the Advanced Project Record allows more room to write) for every project they have completed, as well as a Yearly 4-H Summary.  Some of the forms you need are available as editable Word documents including:
