Livestock Identification
- Market Animals that were weighed in must be verified in the 4-H Online system to ensure all information is correct.
- All other animals (breeding beef, sheep and goats, dairy cattle, dogs, horses, rabbits and bucket bottle calves) must be identified in 4-H Online by May 15th.
- Paper forms may be filled out and brought to the Extension Office, but will not be accepted after May 15th. Absolutely no late Identification forms can be accepted.
- Horse lease agreements must also be turned in by May 15th.

4-H Animal Care and Management Disclosure Statement for Market Animals:
Beef Affidavit (fill out and return with your vet slip at fair)
Swine Affidavit (fill out and return with your vet slip at fair)
Lamb Affidavit (fill out and return with your vet slip at fair)
Meat Goat Affidavit (fill out and return with your vet slip at fair)

- Must be printed, filled out and returned to the Extension Office by May 15th. It is preferable to identify animals through the 4-H Online System. Log on to 4-H Online here: www.4honline.com
