Updated Animal Care & Disclosure Forms (Drug Affidavits)

Due to COOL (Country of Origin Labeling) no longer required in the U.S., the animal care and disclosure statements (drug affidavits) have been updated for beef, meat goat, sheep, and swine. These affidavits are required for State Fair animals, and many county fairs use these as well. Look for the “Updated 2017” in the upper right corner to be sure you have the current ones.
