Animal Identification Forms

If you plan to exhibit any animals at the Greene County Fair or Iowa State Fair, they must be identified by May 15.  This is a firm deadline set by the state.  Any livestock not identified by May 15th will be ineligible to be shown.

The Livestock Identifictaion Period runs from January through May 15.  Animal ID's may be turned in to the Greene County Extension Office at any time during that period.Market beef, swine, and market sheep are all identified at weigh-ins.  However, everything else needs to be identified using one of the following forms.

Some forms are listed with the option of printing a PDF copy or a Word copy.  PDF is the best choice if you want to print it and then fill it out.  If you would prefer to type all of the information, you will want to select the Word version option.

Please note that forms will not be accepted until they are completely filled out!  That means all signatures, etc.  We will not accept them if there is any information missing!  Please do not wait until May 15th to turn in your form!!  If you bring it in at 4:20 pm and are missing even one signature, it will not be accepted and you will miss the opportunity to identify your animal--meaning that you will not be able to show them at the fair!
