ISU
Extension needs the following demographic and contact information
requested below for two reasons. First, as a tax supported
educational arm of the Land Grant system, ISU Extension is
required to report numbers of individuals served to our funders--federal,
state, and local. Nothing personal will be shared, only total
numbers served. Secondly, we need your help in planning for
future educational programs, so we will ask you to complete
an evaluation of this Web site and what you learned through
this program. |
