As you are coming into Fayette on Highway 150, you will see Maple Crest Manor Nursing Home on the EAST side.

• Turn east there onto Clark Street.
• Go down the hill and cross Main Street. Go one more block.
• We are located in the Northeast Iowa Professional Building on your left. Our office is inside the front door on the right.
