Instructions for Submitting Families Success Stories
Families Success Stories are entered via SurveyMonkey at: www.surveymonkey.com
SurveyMonkey form mirrors Success Story format:
- Contact Information
- POW and Quarter
- Title
- Problem Statement
- Extension’s Response
- Impact
Once staff have submitted a Success Story via SurveyMonkey, it is retrieved and sent to JaneAnn Stout for review and approval. Following approval, the story is posted on the Web at: www.extension.iastate.edu/families/success/
Once the Success Story has been posted, staff will receive the story’s unique URL via email. Staff may share this URL with AEEDs, CEEDs, and others as appropriate. Our goal is to post Success Stories within 2-3 weeks provided it is approved on the initial submission. If a story is returned to the author for rewriting, it must be resubmitted via SurveyMonkey.
Success Stories submission requirements:
Full-time field specialists -- 4 per fiscal year
Part-time field specialists -- 2 fiscal per year
CEEDs -- 1 per fiscal year
Additional things to know:
- Survey Monkey does not support tables and charts. All information must be stated in sentences.
- You may copy/paste text from another document into SurveyMonkey.
indicates a REQUIRED field in the survey form that must be completed.
- Staff who wish to have a hard copy of the Success Story are directed to print the Web page.
Benefits of using SurveyMonkey:
The improved procedure leans the Success Stories process by standardizing the information.

- Provides secondary repository for input data
- Significantly reduces time to prepare and post stories to Web
- Reduces total time from submission to Web posting
Feedback from staff surveyed February 2008:
- It's great.....succinct and easy....yeah!!
- I like the method. Not any more difficult than previous version. No suggestions at this time.
- I don’t like that I have to type it in Survey Monkey, then I have to cut and paste to send to CEEDs. It was much easier before just to put in email.

- I like using a “template.” Great to be able to copy and paste from a Word document. Appreciate having the brief explanation prior to each component. Good idea to let us choose which quarter for which the story is being submitted. Previously when I submitted a couple within the same quarter it was easy for all involved to lose track of if a person was up-to-date. No problems but can’t remember if I got the “unique URL” to share with CEEDs, AEED, etc. This part was simpler previously when one message sent it to everyone I wanted to get the story. Now it is a two step process.

- The process is working fine for me … in fact I’m even up-to-date… might even be ahead with my success stories.

- The new format is OK. I still type it up in my old format and then cut and paste into the online form. I email the old format to CEED’s. While there may not be space on line for as many details (if it needs to be shortened for state purposes), there are times the local CEED’s still like all the details – or having the option to share more detail with their council as well as the short and sweet version on the web. This way the CEED’s have an option.
- The format is easy - just wish we could print a copy when we finish instead of having to do in word and paste into the web form. Also - would it be possible to add a spot for names of county directors that notice should go to when the web article is posted. This would let them pick up on items they should share with
Extension Councils or they might even want to link the success story to their web site.
