If you are a new family, and enrolling in 4-H for the first time, please use this help sheet.
Each 4-H member and 4-H leadeder/volunteer must fill out a medical form. You will not be able to type your information in. You will need to print the forms off and write the information in.

You must turn your medical form and the $15 enrollment fee into the Clarke County Extension Office before you will become an active 4-H member. Youth who are in grades K-3rd and leaders/volunteers do not pay an enrollment fee. Please make sure that all signatures are complete before returning the forms to the Extension Office.
