  1. A statement of the problem or issue is developed. Project budget, time line and responsibilities are formalized in a memorandum of agreement.
  2. The population of interest is defined and a source for sample population names and addresses is identified.
  3. The questionnaire is developed and printed.
  4. A draft questionnaire is pre-tested by the community planning group or organization.
  5. Questionnaires are distributed.
  6. Returned questionnaires are coded and a database is created.
  7. Data is analyzed.
  8. A written report is prepared, including distribution frequencies and graphics.
  9. Findings are presented orally to key personnel or committee members.
  10. The group uses the information for planning.

What does a mail survey cost?

Project costs depend on questionnaire length, size of the sample, method of distribution, and how survey responsibilities are shared between CD-DIAL and your committee.
