How do I join 4-H?
If you're wanting more info about a certain club, feel free to contact a leader listed on the map or call the Boone County Extension Office - 432-3882.
+ Once you've picked a club, it's time to fill out the forms. Below are links to our county Enrollment Form and the Iowa 4-H Member Medical Release Form. Print off these forms and turn them in to your 4-H Club Leader or directly to the Extension Office, along with your $35 Program Enrollment Fee.

Members must complete enrollment (forms completed and program development fee paid) by May 1st in order to exhibit at county fair.
