An online survey instrument is now available for use by meeting moderators in reporting the findings of the meeting held in your county. This form contains 17 questions that follow the outline you were sent in advance of your meeting. It should take less than 10 minutes to complete, and when you click the submit button, your results will automatically be entered into a data set that will be used for the final statewide report.
