Submitting ISU Extension Award Nominations


How to submit nominations

  1. Use the ISU Extension Awards Nomination Form provided on this web site.
  2. Write the nomination using the award criteria as stated in the ISU Extension Awards Descriptions of award categories. The Awards Committee recommends the award application writer REVIEW THE SPECIFIC CRITERIA for the award and use the criteria as HEADINGS within the application. This will help the writer focus on accomplishments related to the award criteria. It will assist the committee as they review and compare award applications.
  3. All award nominations must have a 75-word summary (used as a basis for the awards booklet write-up if the nominee is selected).
  4. The complete awards application should contain no more than 825 words, excluding names of nominees and persons submitting the application and a 75 word summary.
  5. Send completed nomination forms via e-mail by 5 p.m., March 1, 2010, to the awards committee at isueawards@iastate.edu
  6. Faxes are NOT acceptable. E-mail the information using the Microsoft Word template and send it as an attachment to the message. Place the name of the award in the subject line (example: New Professional). Please pay attention to length limitations. If you do not receive acknowledgement of your award nomination by March 4th, please contact the awards committee at isueawards@iastate.edu.

Use the Awards Nomination Form provided on this web site. It is aMicrosoft Word document.

The form will ask you to fill in the following fields:

  • Award Category:
  • Name(s) of individual or team nominated and job title(s):
  • Name, title, and home base of person(s) submitting nomination:
  • Title of single effort (if applicable):
  • Summary of award nomination (75 words or less):
  • Award criteria and accomplishments. Remember, use the criteria as HEADINGS within the description of the nominee's accomplishments (825 words or less):
